Then on September 22 you will have the opportunity to meet with a public health nurse for a one-on-one to discuss your health and wellness needs. We can offer help with:
- Registering for a waitlist to find a family physician
- Connecting to a physician while waiting for a family physician
- Finding services and supports in your community
- Assistance registering for the Ontario Seniors Dental Care Program
- Referrals to the Community Connector for social needs (i.e. transportation, meals on wheels, housekeeping services)
